หมายเหตุ :
โปรดตรวจสอบเอกสาร เช็คเกรด และประเภทใบขับแข่งกับเจ้าหน้าที่ ให้ถูกต้องครบถ้วนก่อนดำเนินการชำระค่าธรรมเนียม
Note :
Please submit the documents, license grade, and racing license type with the officer to ensure accuracy and completeness before proceeding with the fee payment.
ชาวไทย
1. สำเนาทะเบียนบ้าน พร้อมรับรองสำเนา
2. สำเนาบัตรประชาชน พร้อมรับรองสำเนา
3. ใบตรวจร่างกาย/ใบรับรองแพทย์
4. ใบตรวจ EKG (กรณีนักแข่งอายุ 45 ปีขึ้นไป)
5. รูปถ่าย 2 รูป (ขนาด 1 นิ้ว หรือ 2 นิ้ว)
6. สำเนาใบขับแข่งปีที่แล้ว (กรณีต่ออายุใบขับแข่ง)
7. หนังสือยินยอมจากผู้ปกครอง (กรณีอายุต่ำกว่าเกณฑ์ที่กำหนด
Foreigners/ชาวต่างชาติ
1. Copy of Passport with Certified True and Correct
2. No Objection Certificate
3. Medical Certificate
4. EKG Certificate (At least 45 years old.)
5. 2 Photos (Size: 1 inch or 2 inch)
6. Parental Consent (if underage)
เอกสารครบถ้วนใช้เวลาประมาณ 7 วัน ในการรับใบขับแข่งตัวจริง / With complete documentation, it takes approximately 7 days to receive your actual racing license.

- Annual Medical Examination / Use for National Competition License only **EKG Certificate (45 years old and older)
- แบบคำขอหนังสือรับรอง (VISA)
